Overview

This S-R-P Rounds discussed several complex pediatric cases involving premature infants and newborns with various gastrointestinal issues. Cases involved bowel obstructions, perforations, and atypical presentations that required careful diagnostic approaches and surgical interventions. Discussion focused on the importance of imaging, stepwise diagnostic procedures, and collaborative decision-making in managing challenging neonatal cases was emphasized.

Case 1. Meconium Plug

This case focused on a premature baby (patient now 38 years old) with bowel dilation and obstruction, including the use of X-rays and contrast enemas for diagnosis. The surgical team explored various potential causes and conditions associated with neonatal bowel obstruction, such as atresia, meconium ileus, and Hirschsprung disease. The discussion emphasized the importance of careful monitoring in premature infants and highlighted that meconium ileus is not always related to cystic fibrosis, with other conditions like intestinal dysmotility disorders also being potential causes.

Case 2. Enteric Duplication Cysts

Baby born at term and presented with bilious vomiting after the mother changed the baby's feeding from exclusive breastfeeding to mixed breastfeeding. The baby was admitted to the NICU and underwent various tests, including an upper and lower GI study and an echocardiogram, which showed atrial septal defect.
