Case Summary

OVERVIEW

Dr. Anna Cristina Castellanos presented a case of a 7-year-old boy with an anorectal malformation and chronic constipation, diagnosed late because of limited access to healthcare. The patient underwent various surgical procedures and is currently undergoing bowel management with rectal dilations and senna. The importance of early diagnosis and potential bladder issues in such cases was discussed, with the recommended time to start Hegar dilations after posterior sagittal anorectoplasty (PSARP) being 14 days post-surgery.

Next Steps

1. Continue monitoring the patient's bowel movements and adjust senna dosage as needed.

2. Urology team to perform urodynamics to further evaluate the patient's bladder function.

3. Follow-up appointments to assess the patient's progress post-surgery and continue rectal dilations as necessary.

4. Medical team to educate the patient's family on proper bowel management techniques and the importance of regular follow-ups.

5. Hospital administration to consider implementing outreach programs to improve early diagnosis of anorectal malformations in underserved areas.
